My grandmother & our entire family received Phenomenal care from the hospice team at First Choice Hospice. Hospice care was somthing that was very foreign for our family, but the moment we decided to transition to hospice care from the first phone call with Irina at First Choice Hospice to the very last visit from Nurse Practitioner Aaliyah when my grandmother passed we felt a major weight lifted and 100% supported every step of the way. The expert care, attention, compassion, respect, and dignity my grandmother received from this team is something we will forever forget and can never fully put into words. We appreciated very much the diversity of the care team and can’t express enough how important it is that you have care providers who look like the patients and families they are caring for and are from the communities they are serving. We felt seen, heard and very supported by your team. If we needed somthing it was at our door immediately, if we had a question Aaliyah & Kim had the answer, if we didn’t know what we needed, what to ask or what to do Aaliyah & Kim had the suggestion for us. We were able to learn how to not fear caring for our mother and grandmother in her final weeks and moments, but embrace the journey and make it a truly beautiful time of unity, care and comfort. Being able to come home to see her Christmas tree, the family dog, and enjoy quality time at home with family while having a full care team, all of her medications, and medical equipment at home was a true blessing. Having her pain and anxiety controlled and being able to rest at home was a true gift. My grandmother was a very independent and very private woman and PCA Lovely was so kind, so professional and so patient with her. The trust she was able to build with my grandmother in such a short time was beautiful to see and helped provide us some respite.
Nurse Practitioner Aaliyah is truly an angel on earth she became part of our very big family knowing everyone by name, listening to us, supporting us, teaching us and always keeping my grandmothers comfort and goals of care at the center of everything we did. She was very good at bringing us all back in and keeping everything on track, but in the most respectful way. Nurse Kim was so thoughtful and knowledgeable, PCA Lovely, Chaplin Emilio, Irena. I can’t thank you enough.
